Eligibility and Application for Medicaid
WHO IS ELIGIBLE TO APPLY UNDER THE MAN PROGRAM?
Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA). These individuals must meet the income and resource standards under the Approved Waiver Plan. SSI recipients are automatically eligible. However, they are required to file an application form.
WHERE CAN I APPLY?
The Medicaid Office is located at Pohnpei Way, Capital Hill, Housing Number 1252. A Medicaid Eligibility Worker is available to assist you with the application process or to answer questions about the program.
DOES IT COST TO APPLY?
There is no enrollment fee, other than what it might cost the individual to obtain a copy of the birth or baptismal certificate to be used as a supporting document.

- Birth Certificate/Passport of the applicant(s)
- Marriage Certificate (If married)
- Legal Guardianship documents (Cour ordered/Notarized Power of Attorney)
- Social Security of applicant(s)
- Bank Statement(s) (Current)
- Credit Union Statement (Current)
- Employment Verification/Certification of Employment
- All Motor vehicle regristration/Certificate of Ownership
- Real Estate Certificates/Deeds/Determination of Ownership.
- Two (2) Current check stub (working applicant(s))
- Current Retirement, U.S. Social Security or Prior Services check stub(s)
- Copy of 1040 Income Tax attached with W-2 Form (Current)
- Current Insurance Policy
- Health Insurance (Medical, Dental, etc.)
- Life Insurance
- Auto Insurance
- Other documents pertaining to income or assets
- Food stamp I.D. / Certification
- Lease Agreement/Rental Receipt(s) (If renting)
- Current Business Gross Revenue Tax (BGRT) (If you have a personal business)
- Proof of evidence that the applicant(s) is a U.S. Citizen/FAS Citizen/Qualified Alien
- Verfication of Pregnancy (Must be issued by the Attending Physician)
- Affidavit of Living Arrangements and Support (Signed and Date)
- All supporting documents that cannot be provided must be supported by a written affidavit.
HOW LONG DOES IT TAKE TO KNOW IF YOU ARE ELIGIBLE?
Processing of the eligibility determination must be made within 45 to 60 days from the filing date of the application. The Eligibility Worker generally could inform an applicant of his/her eligibility based on responses given during the interview with the applicant.
RENEW OF ELIGIBILITY
You can come in and renew your Medicaid eligibility at least 1 month prior to expiration.
