Eligibility and Application for Medicaid

WHO IS ELIGIBLE TO APPLY UNDER THE MAN PROGRAM?

Personal Responsibility and Work Opportunity Reconciliation Act (PRWORA).  These individuals must meet the income and resource standards under the Approved Waiver Plan.  SSI recipients are automatically eligible.  However, they are required to file an application form.

WHERE CAN I APPLY?

The Medicaid Office is located at Pohnpei Way, Capital Hill, Housing Number 1252.  A Medicaid Eligibility Worker is available to assist you with the application process or to answer questions about the program.

DOES IT COST TO APPLY?

There is no enrollment fee, other than what it might cost the individual to obtain a copy of the birth or baptismal certificate to be used as a supporting document.

DOCUMENTS NEEDED TO SUPPORT APPLICATION FORM.

  • Birth Certificate for children.  Passport or birth certificate for parents.
  • Marriage Certificate if married.
  • Social Security card fo applicant and all dependents listed.
  • Banking Activities - type and account no. (Savings, Checking and Certificate of Time Deposit).
  • Credit Union account number.
  • All Motor vehicle regristration or Certificate of Ownership, Real Estate Certificates or Deeds or Determination of Ownership.
  • Copy of Forms 1099
  • Copy of 1040 Income tax return (current).
  • Insurance Policy for automobile, health, dental and life.
  • Other documents pertaining to income or assets.
  • Food stamp I.D. or Certification.
  • Rental receipt or Lease Agreement of renting.
  • With Personal Business, a separate specific release form is required to be signed by the applicant.
    • Address to the bank where the address is
    • Name of Business
    • Then, use the general release form phrases
    • Sign and date by the applicant
  • Pregnancy verfication from Attending Physician
  • CW1, CW2 or Parole in Place (Contract Workers), Green Card/Permanent Resident Card
  • Employment Verification
  • Affidavit of Living Arrangements (Signed and Date)
  • All supporting documents that cannot be produced by the applicant must be supported by an affidavit.

HOW LONG DOES IT TAKE TO KNOW IF YOU ARE ELIGIBLE?

Processing of the eligibility determination must be made within 45 to 60 days from the filing date of the application.  The Eligibility Worker generally could inform an applicant of his/her eligibility based on responses given during the interview with the applicant.

RENEW OF ELIGIBILITY

You can come in and renew your Medicaid eligibility at least 1 month before expiration.
